Help at Home aims to help people return home from hospital in a safe and timely manner, preventing a re-admission to hospital. It also supports adults who are ill or experiencing a crisis at home. It is a service provided by Care Network throughout Cambridgeshire.

Care Network’s Help at Home service offers free practical and emotional support in the home for any adult who has come out of hospital or is experiencing an illness or crisis at home. Anyone, including the person themselves, a concerned family member, a hospital or community-based professional or another  organisation can make a referral for help from this service.

Other services offered by Care Network include:

  • Community Navigator – information and support to access local services and activities to improve personal circumstances and wellbeing.
  • Wellbeing Support – strength-based tailored one-to-one support for people feeling low or anxious to help them develop coping strategies, make personal connections, and build resilience.

Care Network are looking for volunteers to join their team to help people living in Cambridgeshire and Peterborough to remain independent and maintain social connection.
